A ForMed chapter is a branch of our 501(c)(3) nonprofit dedicated to creating community impact and working directly with students.
Chapters range in size and can be formed as school clubs or regional organizations
How to Start a ForMed Chapter
Background Work
If you're a school club, then connect with administration to gain approval. If you're a regional organization, then gather your students and prepare.
Complete the Form
Fill out our chapter registration form and provide our team with information about your proposed chapter, so that we can learn more.
Orientation Meeting
Meet with our team members to discuss plans for your chapter and get oriented to the ForMed family.
